The kid is basically a complete (or near-complete) carbon copy of the other kid she liked who disappeared. Just remove the headband. And her being the borderline psycho yandere she is, instantly believed he was the kid that disappeared just because of that missing headband alone.

Yeah if you look closely you will understand this –
1- she chops the doll to pieces because it now looks like Naoto so if he didn't put the headband on the doll he would have been killed.
2- Naoya is probably Naoto's uncle as Chie and his mom are friends and it is almost impossible to look alike with no blood relation.
3- Chie probably figured out it was Naoto in the end as in the prologue she said phew as she got off the phone. But she wanted to pretend so that she could live in the past.
