I mean, at first, I was “WHAAAAT ???”, but it proved to be a very funny reversal of perspective, it was fun ^^ (And the conclusion is VERY fun, lol ^^)

This is one of my favorite LOL stories. The last two pages make the story, It fact it's almost as funny as the Food Network hentai re-write that's been floating around forever.

The sister and brother (who are not blood related) were running away from home. Although the story doesn't say why, I am guessing they were running away after having been caught being intimate with each other. While they are trying to run away, the kidnappers stop to grab the boy to hold him for ransom. The girl climbs in the car on her own and buckles herself in. Now that the two of them are in captivity together, the girl takes every opportunity she can get to let herself out of her handcuffs, and have sex with her brother. In the one scene towards the end she is holding up a pregnancy test kit, and is quite happy that it shows that she is pregnant with her brothers child, with is where the "grandchild" part of the ending come from. Hope that helps.

I got to admit, the lack of dialogue was a nice touch. They let the images convey emotion, which is good.

Thanks Zathael, now I understand it.
I didn´t recognize the pregnancy test kit. To me it looked like a forceps which she used to open the handcuff at first and therefore I couldn´t understand the story.
I was just too distracted from all the story-unrelevant pictures ;-)
