This one is made manually I think (what a damn tedious work).
But if you want to make a simple one (all thumbs the same size) just use Media Player Classic Homecinema/MPHC. Just go to File-> Save Thumbnails, got all the necessary options, it's pretty straightforward.
Btw, if you happen to don't know it: CCCP is really good in general and even better for animes (compared to VLC for example). It got the MPHC onboard in his own install directory. And it's not a stupid codecpack like KLite that installs itself everywhere and beyond, it behaves like a normal program through the MPHC (or other players if wanted).
