Basically, we’re in the “deeply human, with lots of drama” genre, the series’ female MC doesn’t have long left to live, with a failing pancreas, and in her last times (months? years? No idea) she interacts with a boy who’s her opposite, healthy but dead inside. (Which might explain the series’ English title, “I Want To Eat Your Pancreas“.)
Forget all the drama now, this hentai doujinshi is about enjoying sex while you can ¯\_(ツ)_/¯ The female MC has accepted she’s under a countdown and cares for the present, not the future. Her present being: enjoying sex with a guy she doesn’t dislike
The drawings are heavily censored (numerous black bars), but they remain pretty good: mostly realistic, well done, it’s clearly above the usual fold.

The way the author stretched it out was agonizing, especially after adding some false hope.
“Let us eat and drink, for tomorrow we die.” – 1 Corinthians 15:32b.
There’s much that can be said about that verse from the Bible, which on first glance, seems to reflect the attitude of the female protagonist, but I’ll refrain from doing so at this moment.
Unless I’m mistaken the author just picked an organ at random or something. As far as I know the pancreas is responsible for supplying insulin to transfer nutrients from the bloodstream to various muscles and stuff. So wouldn’t a failing pancreas make her a borderline diabetic who needs exercise and medication to treat it, while when it fails she’d need to take insulin injections or use an insulin pump to supply insulin for the rest of her life? So while a pain in the ass on multiple levels having her pancreas fail shouldn’t mean immediate death as it’s treatable. Now if it was her heart failing that’d be a different story.
